How is this different from a horoscope app?

A horoscope app reads one sign. Kalmanas reads your whole birth chart: the house lords, the yogas, the running dasha, and the divisional chart specific to your question. That is why two people born the same week get very different reports.

What if I do not know my exact birth time?
